Healthcare construction doesn't fail on the build. It fails in the conference room.

Dozens of stakeholders. Clinicians. Infection control. Facilities. Executives. Every one of them with a different definition of "done." Every one of them capable of triggering a design change in month eight that costs six figures and three weeks of schedule.

We talk about scope. They're thinking about workflow.
We lock drawings. They're still debating room adjacency.
We price out casework. They're about to redesign the entire nursing station.

A healthcare contractor is not a builder who works in hospitals. A healthcare contractor is a decision-forcing process that happens to produce a building.

The hidden cost driver on most hospital projects isn't materials or labor. It's late stakeholder alignment. A clinician who didn't get a seat at the table in month two will find one in month eight. Guaranteed. When they do, the RFI log grows, the schedule bleeds, and the contingency disappears faster than anyone wants to explain to the CFO.

The contractors who protect the budget run a structured user-group process before the design is fixed. They lock decision timelines. They document owner sign-offs by department. They make the right conversation happen at the right time.

The ones who skip that process call it "being flexible." The owner calls it a cost overrun.

Is the right answer a more structured contractor-led process, or should hospitals be putting a dedicated owner's rep in that role before a GC ever sets foot on site?
